Dear patient, the following list has been prepared to inform you, and you can save time and costs by reading it. The necessary documents for non-emergency patients staying in the hospital include:
- Registered admission sheet in the insurance book with the date and seal of the attending physician, and for outpatient cases, on the duplicate sheet.
- Completion of the informed consent form by the patient and the physician for patients in the surgery and obstetrics and gynecology departments in emergency cases.
- Valid insurance book and disability institute sheet.
- Presence of a companion for patients requiring surgery.
- For non-emergency cases, children under 18 must be accompanied by a parent, grandparent, or grandparent with an identification card for consent.
- Presence of the spouse for hysterectomy, tube ligation, breast removal, and discharges in non-emergency cases.
- Possession of required medical tests by the doctor, radiographic images, ultrasound, etc., and the doctor's instructions in the clinic.
- Agreed-upon insurance coverage: all basic insurances, including social insurance, general health insurance (treatment services for Iranians, treatment services for employees, support for other groups, and other groups for foreigners), armed forces insurance, disabled insurance, and attachments based on needs.
- Relief Committee: Free payment with a letter and card from the Accident Victims Committee, provided that a report is submitted to the prosecutor's office and emergency in Tehran (police letters are not accepted).
- The following points should be considered for using basic insurance:
- Children under 18 must be accompanied by a legal guardian with an ID.
- To benefit from insurance, sons over 22 years old must obtain a work certificate.
- For daughters over 18, they must carry an ID card.
- Children over two years old must have a copy of the birth certificate.
- For infants under one month old, they must have a copy of the birth certificate with the ID card.
- For the armed forces, they must carry the infant book with a letter recommending hosted patients for more than six hours.

- Hosted patients from the defense forces must go to the following address within 24 hours of staying, carrying the original medical record, the residency order stamped by the treating physician in the green paper, a valid ID card, and the original ID card.
For those with more than one insurance, according to the additional accident insurance law, only one insurer can accept coverage from institutions participating in the mutual coverage system for the ministry.
To benefit from basic health insurance, the following points should be considered:
- The presence of a legal guardian with a legitimate ID for children under 18 for surgical operations.
- To benefit from insurance, children over 22 years old must have a graduation certificate.
- For daughters over 18, they must be accompanied by an ID card.
- Mandatory presence of a copy of the insurance book for children over two.
- For infants under one month old, they must be accompanied by a copy of the birth certificate and the baby's ID card.
- Armed forces personnel must carry the infant book and a recommendation letter for hosted patients for more than six hours.

For patients hosted due to the armed forces, they must contact the following address within 24 hours of the start of their stay, carrying the original medical record, the residency order stamped by the treating physician on the green paper, a valid ID card, and the original ID card.

- Sample examination from 7:30 AM to 12:30 PM.
- Result submission from 1:00 PM to 3:00 PM.
